Knowing when a situation is a genuine emergency — versus something that can wait for a routine appointment — is one of the hardest judgment calls parents face. Erring toward caution is always reasonable; here's a general framework to help sort through it.
Signs That Need Emergency Care Immediately
- Difficulty breathing, bluish lips or face, or grunting with each breath
- Unresponsiveness, extreme lethargy, or a child who is very difficult to wake
- A seizure, especially a first-time one or one lasting more than 5 minutes
- Severe injury, major bleeding, or suspected broken bones with deformity
- Signs of severe dehydration (no urine for many hours, sunken eyes, extreme lethargy)
- Suspected poisoning or ingestion of a harmful substance
- A fever in an infant under 3 months old
- Severe allergic reaction with swelling of the face/throat or difficulty breathing
Situations That Usually Warrant Same-Day (Not Necessarily Emergency) Care
High fever without other red-flag signs, persistent vomiting without dehydration signs, a moderate injury, or a concerning but non-severe symptom often fits better with a same-day pediatric consultation than an emergency room visit — though if in doubt, a phone call to your pediatrician can help decide.
Why "When in Doubt, Get Checked" Is the Right Default
Children, especially young ones, can look relatively well right up until they decompensate quickly — this is different from adults, where decline is often more gradual. A parent's instinct that "something is wrong," even without being able to articulate exactly why, is worth acting on. No pediatrician would rather a worried family stay home than be seen and reassured.
What to Bring to an Emergency Visit
Vaccination records, a list of current medications and known allergies, and a clear timeline of what's happened (when symptoms started, what's been tried) all help the evaluating doctor move faster. If a specific substance was ingested, bring the container or packaging if possible.
