Head Injury in Children: When It's Minor and When It's Not

Bumps and falls are an unavoidable part of childhood, and most head injuries — even ones that cause a dramatic-looking goose egg or a lot of crying — turn out to be minor. Knowing what to watch for afterward is what matters most.

What's Usually Minor

A child who cries immediately after the bump, is consolable within a few minutes, and returns to normal behavior and alertness afterward has typically sustained a minor injury. A bump or bruise on the scalp (sometimes alarmingly large, since scalp injuries bleed and swell more than they look) without other symptoms is usually not a cause for major concern on its own.

What to Watch For After Any Head Bump

Warning Signs That Need Immediate Medical Attention

Loss of consciousness (even briefly), repeated vomiting, a seizure, worsening headache, confusion or unusual behavior, difficulty waking the child, unequal pupil size, clear fluid or blood from the nose or ears, a visible skull deformity or depression, or any head injury in an infant from a significant fall — these all warrant urgent evaluation, often including a CT scan to check for internal injury.

When Imaging Is and Isn't Needed

Not every head bump needs a CT scan — in fact, most don't, and unnecessary imaging exposes children to radiation without benefit. Doctors use established clinical criteria (based on the mechanism of injury, the child's age, and symptoms present) to decide who actually needs imaging versus who can be safely observed. This is a judgment best made by a doctor evaluating the specific child and injury, not by a fixed rule parents apply at home.

Special Caution for Infants

Infants under 1 year, especially under 3 months, are evaluated with extra caution after any fall or head impact, since symptoms can be harder to read in this age group and the threshold for imaging is generally lower.
